Expense Reimbursement Claim
Case: Marion Blodgett, Military Pension
US Pension Office, August 3, 1920
In the Matter of Claim for Reimbursement to Carrie A. Bension
State of Michigan, County of Kalamazoo

Personally appeared before me, a Notary Public in and for the County and State aforesaid, Carrie A. Benson,
who, being duly sworn, declares she is the claimant in this case.  That the soldier above named was her father, and he was taken sick on or about January 1, 1920, from which time until his death he resided at her home, and she gave him her constant attention.  But as the other expenses, undertaker's bill, medicine, cemetery charges, etc., amount to more than the amount of accrued pension, she hereby waives any claim for care and nursing.
They took the remains to Climax, Mich., by automobile for burial, all the automobiles used being donated.
She paid, during the period of his last illness, the sum of $20. for medicine, but took no receipts for those payments.
In the construction of the grave lumber and cement were used, purchased from Climax Lumber Co., and Charles Blodgett.  These bills together with that of George Pierce constitute the cemetery charges of $17.88.
Dr. Pratt, the attending physician, was paid from her father's money, being paid as each visit was made.
      Signed:  Carrie A Benson
Subscribed and sworn to before me this 3rd day of August, 1920, and I certify I have no interest in the matter of this claim.  Signed: Geo. E Foote, Notary Public
